From Crisis to a Place to Call Home

BHS Housing Navigation Services are funded by SAPC's Behavioral Health Bridge Housing (BHBH) program. We provide intensive, tailored support to help homeless individuals with serious behavioral health conditions secure and retain stable housing.

Housing instability and behavioral health conditions often intersect — and resolving one without the other rarely sticks. Through the Behavioral Health Bridge Housing (BHBH) program, BHS Housing Navigators meet clients where they are and walk alongside them through every step of the housing process.

Navigators assist with housing plans, ID recovery, subsidy applications, and move-in support. The goal is to help clients remove barriers and move into permanent supportive housing — and stay there.

How We Help

What Housing Navigation Includes

Intensive, individualized support designed to remove the barriers that keep people from stable housing.

Individualized Housing Plans

Navigators work one-on-one with clients to build a tailored housing plan that fits their needs, goals, and behavioral health journey.

ID Recovery & Documentation

Assistance with obtaining or replacing identification, birth certificates, and other documents required for housing applications.

Subsidy & Benefits Support

Help applying for housing subsidies, vouchers, and public benefits that make stable housing financially possible.

Move-In Support

Hands-on assistance with the move-in process — from inspections and lease signing to securing essential household items.

Retention & Stabilization

Ongoing support to help clients stay housed, navigate landlord relationships, and address barriers as they arise.

Behavioral Health Coordination

Coordinated linkage to mental health, substance use, and primary care services so housing and treatment progress together.

Who We Serve

Eligibility

Housing Navigation Services are available to individuals who are:

  • Experiencing homelessness or housing instability
  • Living with a serious mental illness or substance use disorder
  • Eligible for SAPC-funded behavioral health services in Los Angeles County

Referrals

Referrals are accepted from outreach teams, hospitals, shelters, treatment programs, and community partners. Clients may also self-refer.
